What is eSurvey?
eSurvey is an online application that provides tools for quick and easy
survey creation and dissemination. eSurvey's online results tabulation
capability enables the creator of the survey to view results instantly with a
few easy clicks.

How do I participate and submit
my response to a survey?
Every user in the company can
participate and submit his/her response to company surveys. To do so, simply
click on the 'Participate in Launched Surveys' icon once you have logged in to
the eSurvey module. You will see a list of surveys that have been created by
your company's Superuser or eSurvey administrator. Select the survey that you
would like to participate in by clicking on the 'Open' icon next to the survey.
Complete the survey questionnaire and click the 'Submit' icon. Strict
confidentiality is ensured as your user identity will not be shown on the survey
after it has been submitted.

How do I create a new survey in eSurvey?
Only the Superuser or appointed eSurvey administrator
can create a new survey in eSurvey and edit the survey. If you would like to
create a new survey, please contact your company's Superuser or eSurvey
administrator.
The Superuser or eSurvey administrator can create a new
survey by following these steps:
Once you have logged in to the eSurvey module, click on the 'New Survey' icon.
Enter the title of the new survey and click on the 'Update' icon. The title
of the new survey will be reflected in the list of the existing surveys in the
system.
To create questions in the new survey, select the new survey by checking
against it. Then click on the 'Open' icon and select the 'New Question' icon. A
survey text box will appear for you to key in the survey questions.
After keying in a question, select an answering option for the question by
clicking on the 'Select a Question Type' arrow. You will be shown a preview of
your survey question under the 'Question Preview' section. This will help you to
select the best answering option for each of the survey questions.
If you would like to provide your respondents with an option to indicate
other answers, check against the 'Allow Other Answers' option.
After selecting the answering option for a question, click on the 'Update'
icon, followed by the 'Back to Questions' icon, to store your new question in
the system. To proceed to create your next question, click on the 'New Question'
icon and follow the same instructions as given above (Step 3 to 5).
Once you have completed constructing your survey questionnaire, click on the
'Back to Survey' icon. A list of the existing surveys, including your new
survey, will be shown. You can choose to store the survey in the system for more
changes in the future. Alternatively, you can choose to launch the survey, by
first checking against it and then clicking the 'Launch' icon.

As the Superuser or eSurvey
administrator, how do I edit the title of a survey that is currently stored in
the system?
You can edit the survey title by first checking
against the survey and then clicking on the 'Edit Title' icon next to the
survey. Edit the survey title in the survey text box and click on the 'Update'
icon to submit your changes.

How do I edit and/or delete
survey questions that are currently stored in the system?
You can edit the survey questions by first checking against the survey and then clicking
on the 'Open' icon next to the survey. A list of questions created earlier in
the survey will appear. Click on the 'Edit' icon next to the question that you
would like to edit. Edit the question in the survey text box and click on the
'Update' icon to submit your changes. To delete a survey question, simply select
the question by checking against it. Next, click on the 'Delete' icon and the
survey question will be removed.

How do I edit the answering
options to survey questions that are currently stored in the system?
You can edit the answering options to the questions by first checking
against the survey and then clicking the 'Open' icon next to the survey. A list
of questions created earlier in the survey will appear. Click on the 'Edit' icon
next to the question that you would like to edit. Next, select a new answering
option for the question by clicking the 'Select a Question Type' arrow. Once you
have made your changes, click on the 'Update' icon.

How do I change the question
sequence in a survey that is currently stored in the system? To
change the question sequence in the survey, first check against the survey and
click on the 'Open' icon next to the survey. A list of questions created earlier
in the survey will appear. To move a particular question further down the list,
simply click on the 'Down' arrow shown next to the 'Edit' icon for the question.
Click on the 'Up' arrow to move a question further up the list. Alternatively,
you can change the question sequence by checking against your question and then
clicking on the 'Move Up' or 'Move Down' icon.

How do I launch a survey that is
currently stored in the system?
To launch a survey, simply click
on the 'My Surveys' icon to see the list of surveys in the system. Select the
survey by checking against it and clicking the 'Launch' icon.

How do I check the response to a survey?
You can check the response to a survey anytime before or
after its closing date. To do so, simply click on the 'My Surveys' icon to see
the list of surveys in the system. Select the survey by checking against it.
Next, click on the 'Show' icon. You will be able to see the total number of
respondents to the survey, as well as an online tabulation of the survey
results.

How do I close a survey?
A survey should be closed only on request by the survey
originator. To close a survey, click on the 'My Surveys' icon to see the list of
surveys in the system. Select the survey by checking against it. Next click on
the 'Close' icon, followed by 'Ok'. Once you have done so, the system will
automatically cease accepting respondents to the survey.

How do I delete a survey?
A survey should be deleted only on request by the survey
originator. To delete a survey, click on the 'My Surveys' icon to see the list
of surveys in the system. Select the survey by checking against it, and click on
the 'Delete' icon.

How do I check the current
status of a survey? A user can check the current status of a
survey simply by looking at the status icon located next to each survey.
